"save" in photoshop no longer sends image back to Lightroom

HELP ASAP!!!!  I do all of my edits in Lightroom with the exception of head swaps in photoshop.  With all previous versions I use the "command E" to easily export my photos from Lightroom to photoshop.  Once done with the head swap I use "save" and photo automatically goes back to Lightroom. Easy.  Now when I use "save" the only options are save to computer or save to the cloud.  HELP I need this fixed or a solution to easily have the edited image back to Lightroom.  I have a wedding to edit and need a solution ASAP!!!!

Are you sure you hit "Save" and not "Save as"? "Save" should not give you a dialog at all.

yes I just hit “save” as I have done many many times before without issue. 
the two files I am bringing over from Lightroom (using command E) or jpeg files.

Did you perhaps add a layer while you did the head swap? JPEG does not support layers, so if you send a JPEG from Lightroom to Photoshop and then hit ‘Save’ after adding a layer, Photoshop will ask you where and how to save it because it will need to use another file format (TIFF/PSD) to save a layered file. And Lightroom will not automatically add that new file to the catalog because it did not expect a TIFF/PSD file. So either flatten the file before you hit ‘Save’, or add the new file to the catalog by using ‘Synchronize Folder’ or ‘Import’.
